Please complete and submit this form within 30 days of the completion of your project, no later than December 15, 2022. Failure to submit this report may render you  ineligible for future funding.

The final report is intended to verify that you have:

• Used all grant monies exclusively on the project as described in the grant application budget
• Prominently credited SLC Arts and the New York State Council on the  Arts Decentralization Program on all publicity, advertisements, and  acknowledgements related to the project
• Informed SLC Arts of any  changes in the project as originally described in the grant application  immediately upon making decisions that would affect the project
